A Clinical Study of Breast Masses



Abstract
A clinical and statistical study was made in 408 cases which underwent breast biopsy at the Department of Surgery, Wallace Memorial Baptist Hospital, from January 1991 to December 1992.
1) Among the 408 breast masses, benign breast lesions were noted in 86.5%, malignant breast lesions in 13.5.
2) The most common lesion was fibricystic disease(52.0%), and it was followed by fibrodenoma(23.5%), breast cancer(13.5%).
3) The most prevalent age group of having breast disease was 30 to 39 years(37.0%) and 50 to 59 years(34.5%) in having cancer.
4) The most common clinical manifestation was a palpable mass(93.9%).
5) The lesions were located on the left side in 16 cases(40.8%), and on the right side in 166 cases(40.6%) and bilatoral in 74 cases(18.7%). The most frequent site of masses was upper outer quadreant(54.9%).
6) Most patients visited the hospital within 6months(74.5%)of developing symptoms, and those who visited within 3months were 43.4%.
7) The most common size of a breast mass ranged from 2 to 5cm(64.2%). In the cases of cancer, 4 cases(7.3%) were smaller than 2cm, 37 cases(67.3%) were between 2 to 5cm, 14 cases(25.4%) were larger than 5cm.
8) Mammogram was performed in 236 cases and the positive predictive value was 63.6% and sensitivity 56.0%.
9) We performed Tru-cut needle biopsy by automated biopsy gun in 180 cases(44.4%) and its B.M ratio was 7.5:1. Excisional biopsy was done in 157 cases(38.6%), incisional biopsy was done in 71 cases(17.0%).
10) The results of the diagnostic accuracy in Tru-cut needle biopsy were as follows; accuracy 97.6%, sensitivity 92.0%, specificity 100%, false positive rate 0% and false negative rate 3.4%, 97.6% sensitivity 92.0%, specificity 100%, false
